Chronic Health Conditions in School
Over 25% of students enrolled in kindergarten through grade 12 schools have a chronic health condition. The school nurse coordinates the student’s care in partnership with the parent/guardian, the student, the healthcare team, and the school staff. The school nurse develops emergency plans and trains school staff to implement those plans.
Allergies
- Prior to the start of every school year, parents must request a new Emergency Allergy Plan be completed by the physician and sent to the school.
Asthma
- Prior to the start of the school year, parents of students with severe Asthma must request a new Asthma Action Plan be completed by the physician and sent to the school.
- You must complete the Medication Administration Form each school year for your child's inhalers if they are to be used at school.
Diabetes
- Prior to the start of each year, please have your child's provider send new diabetic orders to the school.
