School District of Belleville Health Office

Welcome to the web site of the Health Office! The mission of the Health Office is to provide a safe and healthy environment so that all students can learn at their optimal level. If you have any health concerns you may contact the School Nurse at 424-3371, extension 250. The School Nurse will work closely with you to create a plan to meet a specific concern you may have with your son/daughter.

Certain forms must be in place for safety and legal reasons. The following links will give you access to specific forms you may need.

Medication consent form

For any over-the counter or prescription medication to be given at school.

Inhaler Release Form

For students in grades 3-12 to carry their inhaler with them at school and any school sponsored activity. Other students may carry their inhaler with arrangement with the School Nurse.

Kindergarten Physical

For all students entering kindergarten. Physician examination form must be on file.

Kindergarten Vision Screening Form

For all students entering kindergarten. Professional eye exam encouraged to be done by eye care professional or physician.

Dental Form

For all students entering kindergarten. Dentist exam record must be on file.

Immunization Card

Required by state law to be on file for all students. Lists all immunizations received by student.

WIAA Sports Form

Physical required for students in grades 7-12 involved in WIAA sports. Available at the High School Office.
