Medication Administration in School

If you would like your child to receive any medication (even over-the-counter medication) at school, you will need to provide the school nurse with the following items:

1. A parent/guardian consent form must be completed each school year- This is your permission for the school staff to administer medication to your child. If a prescription medication will need to be administered, the form must be signed by your child's provider.
2. Prescription medication must come packaged with the prescription label that contains the child's name, name of the medication and dosage, time to be given and practitioner's name.
3. Over-the-counter medication must come to school in the original manufacturer packaging.
